Terms and Conditions

In consideration of the use of the website I, hereby waive any liability Body Design by Alissa, LLC or Alisa Harakal, may have arisen from my participation in any Pilates/fitness program.

Further, I agree to hold harmless Burke Wellness Center, LLC or any other persons and entities involved and/or connected with the Pilates/fitness program in the event of any injuries or damages, physical, personal, or property, which may arise from my participation in the Pilates/fitness program while on the premises of 5276 Lyngate Court, Burke, VA 22015  I agree that I will immediately notify the instructor if I feel injured in any way from a workout session, during or after such workout session.

I hereby request and consent to the performance of Pilates/Fitness Training by Body Design by Alissa, LLC. I understand that frequently, a small amount of soreness may accompany any fitness regimen. I voluntarily participate in the Pilates/fitness program and assume all risks of injury, illness, and/or death. I have read the above consent and understand it.

  • I HAVE NO PREEXISTING CONDITION THAT WOULD AFFECT MY HEALTHY PARTICIPATION IN ANY FITNESS PROGRAM.
  • I AM FULLY RELEASED BY MY DOCTOR TO PARTICIPATE IN ANY PROGRAMS WITH BODY DESIGN BY ALISSA, OR I FULLY RELEASE MYSELF AND TAKE FULL RESPONSIBILITY FOR MY ACTIONS.
  • I OWN OR AM COVERED BY AN ACTIVE HEALTH/ACCIDENT INSURANCE POLICY.
  • I HAVE PREVIOUSLY USED A PILATES REFORMER AND HAVE EXPERIENCE WITH THE EQUIPMENT. I CERTIFY THAT I WILL SAFELY USE THE EQUIPMENT AND HEREBY RELEASE BODY DESIGN BY ALISSA FROM ANY LIABILITY FROM IMPROPER USE OF EQUIPMENT.

If under 18, I will have a legal guardian consent to the above condition.
